Video Permission Form

Video permission forms are required for any child who is videorecorded as part of practicum (e.g., for microteaches).  The practicum student is responsible for obtaining these forms and sharing them with the university supervisor. This form is available in English, French, and Spanish.  If another language is needed, the practicum student should inform the program coordinator.
