Evaluation Forms

University supervisors may use a variety of strategies to record observation notes and data on the practicum student’s performance.  Several forms are provided as a resource for the university supervisor.  These forms are also relevant to the practicum student, as they outline critical skills for EI/ECSE professionals to master.
