Lesson/Session Plan Forms

Weekly plans should be completed for all activities or lessons the practicum student plans, including those that are not observed by the university supervisor.  For each observation by the university supervisor, a lesson (for classroom-based placements) or session (for early intervention or home visiting placements) planning form should be completed.  Several forms are provided, along with examples of how to complete them.  The provided forms should be used.
